What is Home Health Care?

Home health care, simply put is health care delivered in a patient's home. Home health care service can range from in-home nursing care or rehabilitation following a hospital stay, to ongoing assistance with daily living activities such as shopping, meal preparation or bathing.

Skilled Nursing (RN or LVN)
- Evaluation and assessment
- Dressing change and wound care
- Instruction of safe use of oxygen and respiratory equipment
- Supervision of medications
- Restorative care
- Educational programs designed to meet 7 individual health needs
- Insulin administration and diabetic teaching
- Bladder and bowel training

Certified Home Health Aide
Assistance with personal care, meal preparation, ambulation, light housekeeping and other activities of daily living.

Physical Therapy
- Restoration of specific functional loss, range of motion and strength.
- Utilization of adaptive equipment and assistance in rehabilitation.
- Instruction on safe use of assistive devices, safe ambulation, transfers and prevention of falls and injury.

Occupational Therapy
- Therapeutic exercises for activities of daily living skills to facilitate maximum level of functioning.
- Instruction on orthopedic devices and energy conservation exercises.


Speech Therapy
- Assistance in regaining speech abilities which may have been impaired following an illness or accident.
- Instruction on communication and swallowing techniques.
- Intervention for hearing impairment and neurological disorders.

Medical Social Worker
- Assistance with financial and social resources to improve optimum level of care in the home. 
- Assistance with dealing social and emotional factors relating to illness.


Durable Medical Equipment
Arrangement of medical equipment including hospital beds, wheelchairs, commodes, walkers, etc which will be made to meet special needs of the patients. 

Pharmacy
Arrangement of pharmaceutical needs such as IV therapy, insulin, breathing treatments, etc as ordered by the physician.

Payment Arrangements
Who pays for the services?
If you are confined to your home and need intermittent skilled nursing and rehabilitation services, Medicare pays 100% of qualifying services furnished in the home when ordered by a physician.
